보험인정기준 상세내용

■ 고시 개정 전체내용

허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.

- 아 래 -

가. 단순요로감염증

다른 치료방법이 없는 환자에게 사용하며 단독으로 투여 시 3-5일 정도 인정함.

1) 성인 : 1회 100mg을 1일 2회

2) 신장애 환자 (크레아티닌 청소율이 40ml/min 이하의 심한 신기능 장애 환자) : 1회 100mg을 2일 1회

나. 골반내 감염증, 자궁경관염

1) 1차 약제(Ofloxacin경구제 등) 투여로 증상이 호전되지 않는 환자에게 투여하는 것을 원칙으로 함.

2) 타항생제에 내성이 있는 환자, 면역기능이 저하된 환자, 중증감염환자 등의 경우에는 1차 약제로 인정함.

3) 투여기간

7일간 인정하며, 골반내감염증의 경우 증상개선 정도에 따라 14일까지 인정함.

4) 투여용량

가) 성인 : 1회 200mg을 1일 2회

나) 신장애 환자 (크레아티닌 청소율이 40ml/min 이하의 심한 신기능 장애 환자) : 1회 200mg을 2일 1회

국내·외 허가사항 변경 및 교과서 등 관련 문헌을 참고하여 변경된 허가사항을 반영하여 급여기준을 변경함.
