2. 만 24세 이하인 자의 우울병에 투여하는 경우에는 허가사항 중 사용상의 주의사항(경고, 이상반응, 일반적주의 항목 등)에 따른 임상적 유용성이 위험성보다 높은지 신중하게 고려하여 투여하여야 함.
3. 삼환계 항우울제(Amitriptyline HCl, Nortriptyline HCl 등)는 허가사항 범위를 초과하여 아래와 같은 기준으로 투여하는 경우에도 요양급여를 인정함.
- 아 래 -
가. 섬유근육통(Fibromyalgia) 확진 시
나. 과민성 장증후군(Irritable bowel syndrome)에 투여 시
다. 턱관절 장애로 인한 만성통증에 투여 시 : Amitriptyline HCl, Nortriptyline HCl만 인정
※ 섬유근육통 확진은 2010년 미국 류마티스학회 발표 진단기준에 해당되고 섬유근육통진단설문지(FIQ; Fibromyalgia impact questionnaire) 점수가 40점 이상이며, 시각적 아날로그 동통 스케일(pain VAS; pain Visual Analog Scale)이 40mm 이상인 경우에 한함.
라. 대상포진으로 인한 심한 소양증에 투여 시: Amitriptyline HCl, Nortriptyline HCl, Imipramine HCl만 인정
마. 신경병증성 통증에 투여 시 : Amitriptyline HCl, Nortriptyline HCl, Imipramine HCl만 인정
바. 편두통 예방: Amitriptyline HCl만 인정.

제외국 교과서 및 가이드라인에서 신경병증성 통증(대상포진 후 신경통을 포함)에 1차 치료제로 권고하는 점 등을 반영하여 신경병증성 통증에 Amitriptyline, Nortriptyline, Imipramine 에 대해 급여를 인정함.
또한,교과서, 임상가이드라인, 논문 등을 참고하여 Amitriptyline HCl에 대해 편두통 예방에 급여 인정함.
