1. 허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.
- 아 래 -
가. 자살 행동 위험이 있는 조현병 또는 조현정동장애 환자의 자살 행동 위험감소
나. 4주 이내 2가지 이상 약제를 병용 투여하여도 치료효과가 없는 치료저항성 조현병 환자
다. 심한 추체외로계 부작용 또는 만발성 운동장해를 일으키는 환자
2. 허가사항 범위를 초과하여 아래와 같은 기준으로 투여 시에도 요양 급여를 인정함.
- 아 래 -
가. 투여대상
환시(Visual hallucination) 등의 정신과적(Psychiatric) 증상을 동반하는 파킨슨병 환자에서 quetiapine 경구제 투여에 불응하거나 부작용이 발생한 경우
나. 용법·용량
: 저녁에 한 번 투여함
1) 초기용량 : 1일 12.5mg 투여하고 12.5mg씩 주당 2회 증량 가능하며 두 번째 주말에 최대 50mg 이상을 초과하면 안 됨
2) 유지용량
가) 1일 25∼37.5mg 투여.
나) 단, 예외적인 경우에만 1일 50mg 투약을 초과해야 하며 최소 1주일의 기간 동안 50mg 용량으로 치료효과가 불충분한 경우 주의하여 12.5mg씩 증량 가능함. 1일 최대용량 100mg을 초과해서는 안 됨
3) 투여 종료 : 최소 1∼2주 기간 동안 12.5mg씩 점진적으로 감량함
※ 동 약제 투여기간 중 처음 18주 동안은 매 주마다, 그 후에는 적어도 매달 1회 정기적인 백혈구 및 감별 백혈구 검사를 실시하고 검사결과지를 첨부하여야 함.
