보험인정기준 상세내용

■ 고시 개정 전체내용

허가사항 범위 내에서 [일반원칙] 폐동맥고혈압약제 “세부사항” 및 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.

- 아 래 -

가. 대상환자

뉴욕심장협회(NYHA; New York Heart Association) 분류 단계 III, Ⅳ에 해당하는 WHO Group Ⅰ 폐동맥고혈압 환자 중 다음 질환으로 진단이 확인된 환자



- 다 음 -

1) 특발성 폐동맥 고혈압(Idiopathic pulmonary arterial hypertension)

2) 가족성 폐동맥 고혈압(Familial pulmonary arterial hypertension)

3) 교원성혈관질환과 관련된 폐동맥 고혈압(Pulmonary arterial hypertensio nassociated with collagen vascular disease)

4) 선천성 심장질환과 관련된 폐동맥 고혈압(Pulmonary arterial hypertension associated with congenital heart diseases)



나. 투여방법

1) 단독요법

(가) 대상환자 중 우심도자를 통해 폐동맥고혈압이 확진*되고, WHO 기능분류 단계 Ⅳ에 해당하면서, 아래의 지표 중 한 개 이상을 만족하는 경우 동 약제의 단독투여를 인정함.



* 환자의 상태로 인해 우심도자 검사가 불가능한 경우, 심초음파 검사 상 ‘peak tricuspid regurgitation velocity >3.4(m/s)’ 등 폐동맥 고혈압을 확실히 의심할 수 있는 경우를 포함함

(나) 대상환자 중 (가)의 조건에는 해당하지 않으나, 기존의 폐동맥고혈압약제에 반응하지 않거나※ 금기인 경우 동 약제의 단독투여를 인정함.



※ 기존의 폐동맥고혈압약제(Ambrisentan 경구제, Bosentan 경구제, Macitentan 경구제, Sildenafil 경구제, Iloprost 흡입액)에 반응하지 않아 변경 투여하는 경우에는 동 약제가 적정용량에 도달할 때까지 기존치료제는 용량을 서서히 줄이고, 동 제제는 용량을 서서히 늘려 용량을 조절함.

- 용량조절 기간 (최대 3개월) 동안 기존 치료제 및 동 제제 급여인정



2) 병용요법대상환자 및 [일반원칙] 폐동맥고혈압약제 “세부사항” 범위 내에서 투여 시 요양급여를 인정함



다. 치료 효과에 대해 정기적인 평가가 이루어져야 하며, 허가사항 중 주의사항(약물상호작용, 부작용 등)을 고려하여 투여하여야 함.

투여 요법(단독요법 및 병용요법)을 [일반원칙]화하고, 교과서, 임상진료지침, 임상논문, 관련학회 의견, 관련 급여기준 등을 참고하여 일정 조건 충족 시 초기 단독요법에 대해 급여인정.
