보험인정기준 상세내용

■ 고시 개정 전체내용

허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.



- 아 래 -

가. 성인 간정맥폐쇄병

1) EBMT 진단기준(2016년 제정)※에 부합하면서,



※성인 EBMT 진단기준

조혈모세포이식 후 21일 이내 발병 간정맥폐쇄병 (Baltimore 진단기준)

--------------------------------------------------------------------

총 빌리루빈 2mg/dL 이상이면서, 아래 조건을 2가지 이상 만족하는 경우

- 아 래 -

ㆍ통증을 동반한 간비대

ㆍ기저치 대비 체중증가 5% 초과

ㆍ 복수

--------------------------------------------------------------------

조혈모세포이식 후 21일 초과 발병 간정맥폐쇄병

--------------------------------------------------------------------

Baltimore 진단 기준에 부합 또는

조직학적으로 증명된 간정맥폐쇄병 또는

혈류역학적 혹은 초음파적으로 확인된 간정맥폐쇄병 환자로 아래 조건을 2가지 이상 만족하는 경우

- 아 래 -

ㆍ 총 빌리루빈 2mg/dL 이상

ㆍ 통증을 동반한 간비대

ㆍ 기저치 대비 체중증가 5% 초과

ㆍ 복수

* 체중 기저치는 전처치를 시작한 날 측정 체중을 기준으로 하며 측정이 불가능했을 경우 이식 병동에 입원한 날의 체중 사용 가능, 체중 측정은 최적의 이뇨 치료를 받은 후 이루어져야 함



2) 다음 기준을 2개 항목 이상 만족하는 경우

- 다 음 -



항목 기준

첫 임상적 증상 발생으로부터 진단까지의 시간 4일 이하

총 빌리루빈 (mg/dL) 5이상

빌리루빈 kinetics 48시간 이내에 2배 이상 증가

아미노전이효소 (AST 또는 ALT) 정상치의 5배 초과

신장기능 이식 당시 혈청 크레아티닌 수치 대비 1.5배 이상 증가



나. 소아 간정맥폐쇄병 (만 19세 미만)

1) EBMT 소아 진단기준(2017년 제정)※에 부합하면서,



※ 소아 EBMT 진단기준

아래 조건을 2가지 이상 만족하는 경우



- 아 래 -



？ 원인 불명의 소모성, 수혈 불응성 혈소판 감소증

？ 기저치 대비 체중증가 5% 초과 또는 이뇨제 사용에도 불구하고 3일 연속 체중증가가 다른 원인으로 설명되지 않는 경우

？ 간비대 (영상의학 검사)

？ 복수 (영상의학 검사)

？ 총빌리루빈 3일 연속 기저치 대비 증가 또는 72시간 내 2mg/dL 이상



2) 다음 기준을 1개 항목 이상 만족하는 중증인 경우

- 다 음 -



항목 기준

불응성 혈소판감소증 (refractory thrombocytopenia) 7일 초과

총 빌리루빈 (mg/dL) 2 이상

빌리루빈 kinetics 48시간 이내에 2배 이상 증가

아미노전이효소 (AST 또는 ALT) 정상치의 5배 초과

신장기능 이식 당시 혈청 크레아티닌 수치 대비 2배 초과하여 증가

■ 고시 개정 사유

EBMT 가이드라인을 반영하여 소아와 성인의 진단 및 중증도 기준을 분리하고, 성인의 EBMT 중증 진단기준 항목에서 ‘체중증가’를 삭제하고 만족해야하는 항목수를 4개에서 2개로 완화하여 급여 인정함.
