보험인정기준 상세내용

■ 고시 신설 전체내용

각 약제의 허가사항 중 양성 전립선 비대증에 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.

- 아 래 -

가. 투여 시작 시 다음의 1), 2) 조건을 모두 충족하는 경우

- 다 음 -

1) IPSS(International Prostate Symptom Score, 국제전립선증상점수표) 8점 이상

2) 초음파검사 상 전립선 크기가 30ml 이상이거나, 직장수지검사 상 중등도 이상의 양성 전립선비대증 소견이 있거나, 혈청 전립선특이항원(Prostate specific antigen, PSA) 수치가 1.4ng/ml 이상



나. 가에 해당하여 동 약제를 투여하는 동안에는 적어도 12개월 마다 1회 이상 PSA 검사를 시행하여 수치를 평가하고, 이를 기록할 것을 권장함.

■ 고시 신설 사유

교과서, 임상진료지침, 임상연구 문헌 등에서 5알파 환원효소 억제제(이하 5ARI) 복용 시



① 혈청 PSA(prostate specific antigen) 수치가 감소할 수 있고, 이는 전립선암 조기진단을 위한 혈청 PSA 수치에 대한 올바른 해석을 방해할 여지가 있음이 언급되며,

② 5ARI를 투여한 환자 군에서 고위험도 전립선암이 발견될 확률이 증가했다는 연구결과 등을 반영하여 동 약제의 적정투여를 위해 급여기준을 신설함.



또한, 식약처 허가사항에 동 약제 이용 시 PSA 수치에 대한 감시가 필요하다는 점을 고려하여 추적검사를 권고함
