1. 허가사항 및 「암환자에게 처방ㆍ투여하는 약제에 대한 요양급여의 적용기준 및 방법에 관한 세부사항」 범위 내에서 투여 시 요양급여 함을 원칙으로 함.
2. 허가사항의 범위(효능ㆍ효과 등)를 초과하여 아래와 같은 기준으로 투여한 경우에도 요양급여를 인정함.
- 아 래 -
가. 전신성홍반성낭창(SLE)
나. 타 치료에 실패한 류마토이드 관절염에 투여 시
다. 성인의 신증후군(Minimal change) : Steroid 치료후 사용하는 경우
라. 특발성 혈소판 감소성 자반병(ITP) : 스테로이드치료, 비장 절제 후에도 호전되지 않는 경우
마. 베쳇증후군
바. 급성 진행성 사구체신염, 막성 신증후군, 원발성 초점성 분절성 사구체신염
사. 중증 초그-스토라우스 증후군(Churg-Strauss 증후군)
아. 괴사성 공막염
자. 다발성 근육염 및 피부근육염 : Steroid 치료에 실패하였거나 투여가 불가능한 경우
차. 동종조혈모세포이식 후 이식편대숙주질환(GVHD)의 예방을 위하여 투여하는 경우
카. 자가면역성뇌염(Autoimmune encephalitis) 환자 중 Human immunoglobulin G(IVIg)에 불충분한 반응을 보이거나 금기 또는 부작용 등으로 투여할 수 없는 경우
3. 허가사항을 초과하여 혈우병A, B 항체 환자의 면역관용요법에 투여하고자 건강보험심사평가원의 사전승인을 받은 경우에도 요양급여를 인정함. 이 경우 사전승인을 위한 위원회 구성, 사전승인의 기준, 절차 및 방법 등 필요한 사항은 건강보험심사평가원장이 정함.
