■ 고시 개정 전체내용
1. 허가사항 범위 내에서 투여 시 요양급여함을 원칙으로 하며, 보조생식술에 난포자극호르몬 (FSH)과 병용 투여한 경우 병용 약제의 총 용량은 600IU/day 까지 요양급여를 인정함.
- 단, Follitropin delta와 병용 투여할 경우에는 Follitropin delta 허가사항 중 사용상의 주의사항(일일 용량으로 이 약 10마이크로그램을 투여받은 환자의 대부분에서 일일 용량으로 폴리트로핀알파 150IU 투여 시 얻어진 것과 유사한 난소반응을 나타냄.)을 참고하여 병용 약제의 인정 총 용량 범위내에서 투여하여야 함.
2. 허가사항 범위(용법ㆍ용량)를 초과하여 아래와 같은 기준으로 투여 시에도 요양급여를 인정함.
- 아 래 -
가. 무배란성 여성불임증 치료
1) 1cycle 당 최대 18앰플(75IU 기준) 정도 투여함을 원칙으로 하며(월단위),
가) 초기용량 투여로 배란유도에 실패한 경우에는 허가사항 범위를 초과(용법ㆍ용량)하여 1일 최대 6앰플(75IU 기준)까지 투여하는 경우에도 요양급여를 인정함
나) 1cycle당 최대 72앰플(75IU기준)을 초과할 수 없음
나. 보조생식술
○ human chorionic gonadotrophin 주사제 투여 당일에 투여한 경우
3. 상기 1. 2. 중 보조생식술에 투여시에는 「보조생식술 급여기준」 범위 내에서 요양급여를 인정하며, 비급여로 실시한 경우에는 약값 전액을 환자가 부담토록 함.

■ 고시 개정 사유
○ 국내·외 허가사항, 관련문헌(교과서, 임상진료지침, 임상연구문헌 등) 및 학회(전문가)의견 등을 참조하여 hMG와 Follitropin delta의 병용 투여 인정
- 단, Follitropin delta는 기존약제들과 단위가 다른 점 고려하여 hMG와 병용투여 시 동 약제의 허가사항을 참고하여 병용 약제의 인정 총 용량 범위내에서 투여하도록 함
