■ 고시 개정 전체내용



1. 허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.

- 아 래 -

○ 신생아 호흡곤란 증후군: 다음의 조건이 모두 충족되는 경우

- 다 음 -

1) 환아의 호흡곤란증상이 뚜렷하고,

2) 흉부방사선 소견: 호흡곤란증후군(RDS)의 특징적 소견(양 폐에 미만성의 망상과립상음영, 공기. 기관지음영과 연무상이 출현하는 경우)이 있으며,

3) 기계적 환기 요구도: 적정한 혈중산소분압(50-80mmHg)을 유지시키기 위한 인공호흡기의 흡입 산소농도가 30%(FiO2≥0.3)이상인 경우



※ 인공호흡기(침습적 방법 및 비침습적 방법 포함) 사용은 환아의 전반적인 상태(체중, 폐기능 상태, 기타 신체 장기들의 상태 등)를 고려하여 의학적 판단하에 시행되어야 함.



2. 미숙아에 대한 조기투여 요법은 아래와 같이 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.

- 아 래 -

가. 투여대상: 출생체중 1,250g이하 또는 재태기간 30주미만의 미숙아

나. 투여방법: 출생 후 2시간 이내에 1회 투여



3. 허가사항 범위를 초과하여 아래와 같은 기준으로 투여시 요양급여를 인정함.

- 아 래 -

○ 태변흡인 증후군: 다음의 조건이 모두 충족되는 경우

- 다 음 -

가. 환아의 호흡곤란증상이 뚜렷하고

나. 흉부방사선 소견: 태변흡인증후군(MAS)의 특징적 소견(양 폐의 과환기, 폐문 주위의 불규칙한 음영 증가, 횡격막의 수평화)이 있으며,

다.기계적 환기 요구도: 적정한 혈중산소분압(50-80mmHg)을 유지시키기 위한 인공호흡기의 흡입 산소농도가 30% (FiO2≥0.3)이상인 경우



4. 동 약제를 투여할 수 있는 요양기관 시설기준 및 조건

가. 신생아 호흡곤란 증후군 또는 태변흡인 증후군 치료 경험이 있는 진료의사의 책임 하에 사용되어야 하며 신생아 집중치료에 필요한 숙련된 근무자들이 있어야 함.

나. 보육기, 인공호흡기, 혈중가스검사기(혈액가스분석기, 경피산소분압기(TcPO2) 및 산소포화도 측정기(SPO2) 중 1개)와 심폐감시장치 등 신생아 집중치료에 필요한 의료장비가 있어야 함.

다. 상기 가, 나 조건이 미비한 요양기관에서는 상기 가, 나 조건이 충족되는 요양기관으로 즉시 이송하여야 함. 다만, 환아소생을 위한 응급 조치로 투약 할 경우에는 동 약제의 투약기준에 따라 1회에 한하여 투여할 수 있음.



5. 환아의 전반적인 상태(체중, 폐기능 상태, 기타 신체 장기들의 상태 등)를 고려하여 투여하여야 함.

■ 고시 개정 사유

○국내·외 허가사항, 교과서, 가이드라인, 임상논문, 학회(전문가) 의견 등을 참조하여 폐계면활성제의 급여대상을 확대하고 품목 급여삭제에 따른 고시 제목 현행화 함.
