보험인정기준 상세내용

■ 고시 개정 전체내용



1. 각 약제별 허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함

- 아 래 -

가. 투여대상

1) 3mg, 5mg, 10mg 경구제 및 87.5mg, 175mg 패취제

가), 나) 조건을 동시에 충족하는 알츠하이머 형태(뇌혈관 질환을 동반한 알츠하이머 포함)의 경증(mild), 중등도(moderate), 중증(severe) 치매증상

가) 간이정신진단검사(MMSE; Mini Mental State Exam) 26점 이하

나) 치매척도검사

(1) CDR(Clinical Dementia Rating) 1∼3 또는

(2) GDS(Global Deterioration Scale) stage 3∼7

2) 23mg 경구제

가), 나) 조건을 동시에 충족하는 알츠하이머 형태(뇌혈관 질환을 동반한 알츠하이머 포함)의 중등도·중증 치매증상

가) 간이정신진단검사(MMSE; Mini Mental State Exam) 20이하

나) 치매척도검사

(1) CDR(Clinical Dementia Rating) 2∼3 또는

(2) GDS(Global Deterioration Scale) stage 4∼7



나. 투여용량 및 기간

1) 3mg 경구제

가) 소화기계 이상반응 감소 목적으로 필요 시 초기 용량을 1일 1회 3mg으로 시작할 수 있으나 1~2주를 초과하여 사용하지 않는 것을 원칙으로 함.

나)저체중(BMI<18.5kg/m2)인 85세 이상 여성환자에서 1일 1회 3mg 지속투여가 필요한 경우 다. 평가방법에 따라 재평가하여 계속투여 여부를 결정함.

※ 가), 나)에 해당하여 1일 1회 3mg로 투여 시작 후 6~8주를 초과하여 사용이 필요한 경우에는 투여소견서를 제출하여야 함.



다. 평가방법

6-12개월 간격으로 재평가하여 계속투여 여부를 결정하며, 재평가에서 MMSE의 경우에는 26점(3mg*, 5mg, 10mg 경구제 및 87.5mg, 175mg 패취제) 또는 20점(23mg 경구제)을 초과하여도 지속 투여를 인정함.

* 저체중(BMI<18.5kg/m2)인 85세 이상 여성환자에 한함



○ 다만, MMSE 10점 미만이고 CDR 3 (또는 GDS 6∼7)인 중증 치매인 경우, 6-36개월 간격으로 재평가할 수 있음.

○ 노인장기요양보험법령에 따른 장기요양 1등급인 경우, 장기요양인정 유효기간까지 재평가없이 계속투여 여부를 결정할 수 있음(「노인장기요양보험법」 제17조의 장기요양인정서를 제시해야 함).



2. 동 제제와 Memantine경구제 병용 시 알츠하이머 형태(뇌혈관 질환을 동반한 알츠하이머 포함)의 중등도·중증 치매증상으로 각 약제의 급여기준에 적합한 경우 요양급여를 인정함.



3. 동 제제와 Ginkgo biloba extract제제 병용 시 각 약제의 허가사항 범위 내에서 투약비용이 저렴한 1종의 약값 전액을 환자가 부담토록 함.

○ 식약처 허가사항, 교과서, 가이드라인, 임상연구문헌, 전문가 의견 등을 반영하여 donepezil 3mg 경구제 투여기간 및 평가방법을 명확히 함.
