2. 주사제를 조혈모세포이식 환자에 사용할 경우와 경구제를 만성 류마티스성 관절염에 사용할 경우에는 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.
- 아 래 -
가. 주사제를 조혈모세포이식 환자에 사용할 경우
1) 비혈연간 동종 조혈모세포이식 환자
: 이식 1일 전부터 이식 후 20일까지 (이식일 포함 총 22일) 인정하며, 이후 이식편대숙주질환(GVHD) 악화가 없는 경우에는 경구제로 변경 투여하여야 함.
2) 타 약제에 대하여 치료불응성(Refractory)이거나 부작용이 심한 동종조혈모세포이식 환자
: 경구 투여가 불가능한 경우 인정함.
3) 경구투여 하다가 다시 주사제로 전환하는 경우에는 "요양급여의 기준에 관한 규칙[별표1] 3-나 주사제 지급원칙“에 따라 타당한 경우 인정함.
나. 경구제를 만성 류마티스성 관절염에 사용할 경우
: DMARDs(Disease Modifying Anti-Rheumatic Drugs)로 6개월 이상 치료 후에도 효과가 없는 중증(Refractory) 류마티스관절염에 투여 시 요양급여를 인정함.
3. 허가사항 범위를 초과하여 아래와 같은 기준으로 투여한 경우에도 요양급여를 인정함.
- 아 래 -
가. 췌장, 폐, 심장, 소장, 손 및 팔 이식의 경우
나. 중증근무력증 : Azathioprine 또는 Steroid제에 치료효과가 없거나 부작용 등으로 투여할 수 없는 경우에 투여 시 요양급여를 인정함.
다. 소아 난치성 신증후군(일반형 경구제만 해당): 스테로이드 의존성(빈번 재발 포함) 또는 스테로이드 저항성 신증후군에 cyclosporine의 부작용이 심하여 투여가 불가능한 경우
라. 다발성근염 또는 피부근염
4. 허가사항 범위를 초과하여 아래와 같은 기준으로 투여 시 약값 전액을 환자가 부담토록 함.
