동 약제 투약개시일 기준 6개월 이내 다음의 국소치료제 및 전신 면역억제제 투여이력이 확인되어야 함.
-다 음-
가) 국소치료제(중등도 이상의 Corticosteroid 또는 칼시뉴린 억제제)를 4주 이상 투여하였음에도 조절되지 않는 경우
나) 이후 전신 면역억제제(Cyclosporine 또는 MTX)를 3개월 이상 투여하였음에도 반응(EASI 50%이상 감소)이 없거나 부작용 등으로 사용할 수 없는 경우
- 단, 65세 이상, 심혈관계 고위험군, 악성 종양 위험이 있는 경우 기존 치료제(생물학적 제제)에 반응하지 않거나 내약성이 없는 경우에 한하여 동 약제를 사용하여야 함.
3) EASI(Eczema Area and Severity Index) 23 이상
나. 평가방법
1) 투여 후 16주차 평가(최초 평가): EASI 75%이상 감소 시 인정
2) 이후 6개월마다 평가: 최초 평가결과 유지 시 인정
다. 교체투여
1) 동 약제에 효과가 없거나 부작용 등으로 지속 투여가 불가능한 경우 생물학적 제제로 교체투여를 인정함(투여소견서 첨부)
- 단, 65세 이상, 심혈관계 고위험군, 악성 종양 위험이 있는 경우는 제외
※ 생물학적 제제: Dupilumab, Lebrikizumab, Tralokinumab 주사제
2) 교체한 약제는 최소 6개월 투여 유지를 권고함.
3) 동 계열(JAK 억제제) 내 교체투여는 인정하지 않음.
라. 진료과 및 자료제출
1) 동 약제는 아토피 관련 진료과(피부과, 알레르기내과, 소아알레르기호흡기) 전문의가 처방하여야 함.
2) 상기 가. 투여대상 및 나. 평가방법 관련 객관적 자료(약제투여 과거력, EASI 산출근거, 환부 사진 등)를 제출하여야 함.
3. 성인의 중증의 활동성 강직성 척추염
가. 투여대상
두 가지 종류 이상의 비스테로이드항염제(NSAIDs) 혹은 DMARDs로 3개월 이상 치료를 하였으나 치료효과가 미흡하거나, 상기 약제들의 부작용 등으로 치료를 중단한, 중증의 활동성 강직성 척추염 환자
- 단, 65세 이상, 심혈관계 고위험군, 악성 종양 위험이 있는 경우 기존 치료제(생물학적 제제)에 반응하지 않거나 내약성이 없는 경우에 한하여 동 약제를 사용하여야 함.
※ 중증의 활동성 강직성 척추염:
Modified New York criteria 1984(아래 참조)를 근거로 하여, 방사선학적 기준(Radiologic criteria)과 2개 이상((1)항은 반드시 포함)의 임상적 기준(Clinical criteria)을 동시에 만족하며, Bath 강직성 척추염의 질병 활동 지수(BASDAI: Bath Ankylosing Spondilitis Disease Activity Index)가 적어도 4 이상인 경우
가) 방사선학적 기준(Radiologic criteria) 천장골염: 양측성 Grade 2이상, 편측성 Grade 3 혹은 4(Sacroilitis: Grade ≥2 bilateral or Grade 3 or 4 unilateral.)
나) 임상적 기준(Clinical criteria )
(1) 운동 시 호전되나, 휴식으로는 호전되지 않는 3개월 이상의 요통과 강직(Low back pain and stiffness for more than 3months, which improves with exercise but is not relieved by rest.)
(2) 시상면과 전두면 모두에서 요추골 운동의 제한(Limitation of motion of the lumbar spine in both the sagittal and frontal planes.)
(3) 흉부팽창의 제한(Limitation of chest expansion.)
나. 평가방법
1) 16주 투여 후 평가(최초 평가): BASDAI가 50% 또는 2(Scale 0-10) 이상 감소한 경우 인정
2) 이후 6개월마다 평가: 최초 평가결과 유지 시 인정
다. 교체투여
동 약제에 효과가 없거나 부작용으로 투약을 지속할 수 없는 경우에는 투여이력이 없는 다음 약제로 교체투여를 인정함(투여소견서 첨부)
7. 장기처방 시 1회 처방기간은 퇴원할 경우 및 외래의 경우에는 최대 30일분까지로 함. 다만, 최초 투약일로부터 24주 이후에 안정된 질병활동도를 보이고 부작용이 없는 환자의 경우 최대 60∼90일분까지 인정함.
8. 동 약제의 허가사항 중 ‘사용상 주의사항(금기 등)’을 반드시 참고하여 투여하여야 함.
9. 동 약제를 사용하는 경우에는「TNF-α inhibitor 사용 시 잠복결핵 치료지침」을 따라야 함.

○ 국내·외 허가사항, 교과서, 가이드라인, 임상논문, 학회 (전문가) 의견 등을 참조하여, 중증의 강직성 척추염에서 TNF 억제제 및 IL-17 억제제와 동일한 투여대상 기준을 적용하는 것으로 급여기준 확대하고, 성인의 활동성 및 진행성 건선성 관절염에 Tofacitinib 경구제의 급여기준 확대에 따라 교체투여 약제 추가 명시함.
○ 궤양성 대장염에 Guselkumab 주사제(품명: 트렘피어프리필드시린지 등)가 급여 확대됨에 따라 교체투여 급여기준에 해당 성분명을 추가하고, 급여기준 전반에 걸쳐 가독성 향상을 위한 문구 및 용어 등 정비함.
