1. 각 약제별 허가사항 범위 내에서 아래와 같은 기준으로 투여 시 요양급여를 인정하며, 동 인정기준 이외에는 약값 전액을 환자가 부담토록 함.
- 아 래 -
가. 성인의 활동성 및 진행성 류마티스관절염
1) 투여대상
ACR/EULAR 진단기준(2010년 제정)에 부합하는 성인 류마티스 관절염 환자 중 다음 한 가지에 해당하고 두 가지 종류 이상(MTX(methotrexate) 포함)의 DMARDs(Diease Modifying AntiRheumatic Drugs)로 6개월 이상(각 3개월 이상) 치료하였으나, 치료효과가 미흡하거나 상기 약제들의 부작용 등으로 치료를 중단한 환자 (다만, MTX 사용이 불가능한, 간질환 또는 신부전 등의 경우에는 MTX를 제외한 두 종류 이상의 DMARDs 사용)
- 다만, 65세 이상 환자, 심혈관계 고위험군 환자, 악성 종양 위험이 있는 환자에서는 기존 치료제(TNF 억제제 등 생물학적 제제)에 적절히 반응하지 않거나 내약성이 없는 경우에 한하여 이 약을 사용하여야 함.
가) 동 약제를 6개월간 사용 후 평가시 DAS28이 1.2 이상 감소한 경우 추가 6개월간의 사용을 인정함.
나) 이후에는 6개월마다 평가를 하여 첫 6개월째의 평가 결과가 유지되면 지속적인 투여를 인정함.
3)종양괴사인자알파저해제(TNF-α inhibitor: Adalimumab, Etanercept, Golimumab, Infliximab 주사제) 또는 Abatacept, Tocilizumab 주사제, Baricitinib, Upadacitinib, Filgotinib 경구제에 효과가 없거나 부작용으로 투약을 지속할 수 없는 경우 또는 복약순응도 개선의 필요성이 있는 경우(교체한 약제는 최소 6개월 투여를 유지하도록 권고함)에 동 약제로 교체투여(Switch)를 급여로 인정하며, 이 경우에는 교체투여에 대한 투여소견서를 첨부하여야 함.
나. 소아 특발성 관절염
1) 투여대상
다음 가), 나) 조건을 동시에 충족하는 경우
가) ILAR 진단기준(2001년 제정)에 부합하는 소아 특발성 관절염 환자로서 다음에 해당하는 경우
- 다 음 -
(1) 다관절형 관절염(2-17세): 5개 이상의 부종관절이 있는 경우
(2) 확장성 소수 관절염(2-17세)
(3) 건선성 관절염(2-17세)
나) 1종 이상의 생물학적 제제에 반응이 불충분하거나 부작용 등으로 치료를 중단한 환자
2) 평가방법
가) 동 약제를 6개월간 사용 후 평가 시 활성 관절수(부종 관절 등)가 최초 투여시점 보다 30% 이상 감소된 경우 추가 6개월간의 사용을 인정함.
나) 이후에는 6개월마다 평가하여 첫 6개월째의 평가결과가 유지되면 지속적인 투여를 인정함.
3) 동 약제에 효과가 없거나 부작용으로 투약을 지속할 수 없는 경우 또는 복약순응도 개선의 필요성이 있는 경우 (교체한 약제는 최소 6개월 투여 유지 권고) 이전에 투여한 적이 없는 다른 생물학적 제제로 교체투여(Switch)하는 경우 급여를 인정하며, 이 경우에는 교체투여에 대한 투여소견서를 첨부하여야 함.
4) 허가사항 범위를 초과하여, 17세 이전에 소아 특발성 관절염(juvenile idiopathic arthritis)으로 진단된 후, 17세 이후 동 약제 급여기준에 적합하여 동 약제를 사용한 경우에도 요양급여를 인정함. 다만, 소아 특발성 관절염 투여대상 해당 연령 이전에 동 약제를 투여하는 경우에는 사례별 심사함.
다. 성인의 중등도-중증 활동성 궤양성 대장염
1) 투여대상
Corticosteroid나 6-Mercaptopurine 또는 Azathioprine 등 보편적인 치료 약제에 대해 적정한 반응을 나타내지 않거나 내약성이 없는 경우 또는 상기 약제가 금기인 중등도-중증의 궤양성 대장염 환자
- 다만, 65세 이상 환자, 심혈관계 고위험군 환자, 악성 종양 위험이 있는 환자에서는 기존 치료제(TNF 억제제 등 생물학적 제제)에 적절히 반응하지 않거나 내약성이 없는 경우에 한하여 이 약을 사용하여야 함.
※ 중등도-중증의 궤양성 대장염 환자
※ 궤양성대장염 활동평가를 위한 메이요 점수시스템(Mayo Scoring System for Assessment of Ulcerative Colitis Activity)
배변빈도(Stool frequency)
0 = Normal no. of stools for this patient
1 = 1 to 2 stools more than normal
2 = 3 to 4 stools more than normal
3 = 5 or more stools more than normal
Subscore, 0 to 3
직장출혈(Rectal bleeding)
0 = No blood seen
1 = Streaks of blood with stool less than half the time
두 가지 종류 이상의 비스테로이드항염제(NSAIDs) 혹은 DMARDs로 3개월 이상 치료를 하였으나 치료효과가 미흡하거나, 상기 약제들의 부작용 등으로 치료를 중단한, 중증의 활동성 강직성 척추염 환자
- 다만, 65세 이상 환자, 심혈관계 고위험군 환자, 악성 종양 위험이 있는 환자에서는 기존 치료제(TNF 억제제 등 생물학적 제제)에 적절히 반응하지 않거나 내약성이 없는 경우에 한하여 이 약을 사용하여야 함
※ 중증의 활동성 강직성 척추염:
Modified New York criteria 1984(아래 참조)를 근거로 하여, 방사선학적 기준(Radiologic criteria)과 2개 이상((1)항은 반드시 포함)의 임상적 기준(Clinical criteria)을 동시에 만족하며, Bath 강직성 척추염의 질병 활동 지수(BASDAI: Bath Ankylosing Spondilitis Disease Activity Index)가 적어도 4 이상인 경우
가) 방사선학적 기준(Radiologic criteria) 천장골염: 양측성 Grade 2이상, 편측성 Grade 3 혹은 4(Sacroilitis: Grade ≥2 bilateral or Grade 3 or 4 unilateral.)
나) 임상적 기준(Clinical criteria )
(1) 운동 시 호전되나, 휴식으로는 호전되지 않는 3개월 이상의 요통과 강직(Low back pain and stiffness for more than 3months, which improves with exercise but is not relieved by rest.)
(2) 시상면과 전두면 모두에서 요추골 운동의 제한(Limitation of motion of the lumbar spine in both the sagittal and frontal planes.)
(3) 흉부팽창의 제한(Limitation of chest expansion.)
2) 평가방법
가) 동 약제를 16주간 사용 후 평가하여 BASDAI가 50% 또는 2(Scale 0-10)이상 감소한 경우 추가 6개월의 투여를 인정함.
나) 이후에는 6개월마다 평가하여 첫 16주째의 평가결과가 유지되면 지속적인 투여를 인정함.
3) 동 약제에 효과가 없거나 부작용으로 투약을 지속할 수 없어 이전에 투여한 적이 없는 TNF-α inhibitor (Etanercept, Adalimumab, Infliximab, Golimumab 주사제) 또는 Ixekizumab, Secukinumab 주사제, Upadacitinib 경구제로 교체투여(Switch)하는 경우 급여를 인정하며, 이 경우에는 교체투여에 대한 투여소견서를 첨부하여야 함.
마. 건선성 관절염
1) 투여대상(모두 만족)
가) 각 3개 이상의 압통 관절과 부종 관절
- 1개월 간격으로 2회 이상의 연속 측정 결과여야 함.
나) 선행치료
두 종류 이상의 DMARDs로 총 6개월 이상(각 3개월 이상) 치료하였으나, 치료효과가 미흡하거나 부작용 등으로 치료를 중단한 경우
- 다만, 65세 이상, 심혈관계 고위험군, 악성 종양 위험이 있는 경우 기존 치료제(TNF 억제제 등 생물학적 제제)에 반응하지 않거나 내약성이 없는 경우에 한하여 동 약제를 사용하여야 함.
2) 평가방법
가) 3개월 투여 후 평가(최초 평가): 활성 관절수 30% 이상 감소 시 인정
나) 이후 6개월마다 평가: 최초 평과결과 유지 시 인정
3) 교체투여
가) 다음 약제 투여에도 효과가 없거나 부작용으로 지속 투여가 불가능한 경우, 복약 순응도 개선의 필요성이 있는 경우 동 약제로 교체투여를 인정함(투여소견서 첨부)
